It all starts with filling up a Change request/ Correction form by the applicant. Depending on your preferences, either you can download this form from the official websites of UTI Technology Services Ltd (UTITSL), the Income Tax department of India or National Securities Depository Ltd (NSDL). Alternatively, obtaining a hard copy from any of the nearest branches of UTITSL, IT and NSDL is possible again.
Following the first step, you will need to fill up all particulars about the lost/destroyed PAN card in the form.
